DOCUMENTARY REQUIREMENTS

The list is obtained from the VFS website.

  1. Application Form

    download and complete the application form DO NOT leave blanks in the form. Indicate “NA” if answer is not applicable

  2. Photo

    One (1) recent passport-sized or 2x2 inch colored ICAO standard photo with white background Your ears should be exposed and with collared shirt/blouse

  3. Travel Document (Passport)

    Old (if available) and current valid passport (expiring date must be three months longer than visa requested period Immigration Officers require it to have at least 6 months validity

  4. Passport Copy

    Photocopy of Biographical page of the valid passport

  5. Proof of Travel Arrangements

  6. Proof of Financial Means

    Bank certification, bank books, personal bank statements showing regular income, credit card statements, or balance covering the last six months For Minor Proof of parents’ or legal guardians’ economic means

  7. Proof of Ties in the Philippines

    • Proof of social security contributions

    • copy of real estate property (title-deed)

    • Proof of family ties in the Philippines (e.g. birth certificate of children or marriage certificate) which should be issued by the Philippine Statistics Authority (PSA)

  8. Proof of Occupation

    If EMPLOYED

    • Certificate of employment

    • Certificate of leave of absence

    • Latest income tax return


    If SELF-EMPLOYED:

    • Proof of company registration issued by the Department of Trade Industry (DTI) or Securities Exchange Commission SEC registration of business

    • Latest Income tax return

    • Business financial statement


    If PUPIL/STUDENT:

    • Proof of enrollment

    • certificate of leave of absence if traveling during school year.

  9. Health Insurance

    Travel Health Insurance of EUR 30,000 minimum coverage (with 15 days of allowance beyond travel period) and valid throughout Schengen area for emergency hospitalization and repatriation expenses. The insurer has to be from the following list of accredited companies

    Make sure to purchase an insurance with 15 days allowance and to only get from the accredited list as the Italian VFS is known to refuse acceptance of insurance not in the list

  10. Proof of Accommodation

    Confirmation of hotel booking, hotel voucher or letter of promise of accommodation by the host (invitation letter/DICHIARAZIONE GARANZIA E/O ALLOGGIO)

  11. Visa Fee

    90,- Euro (adults) and 40,- Euro (minors from 6-12 years)

    No visa fee for minors below 6 years and for spouses or minor children of EU nationals traveling or accompanying the EU national to Italy

    plus service fee of VFS, payable in Philippine Pesos on the application day at VFS.

 

CONSULAR FEES

Schengen visa: Php  5,998.00
Schengen visa for ages between 6-11 years: Php 2,999.00

In addition to the fees paid for the visa, an additional VFS charge of PHP 1866 (including VAT) is applied for each visa application.
All Fees would be collected at the application submission counter by CASH and Card Payments.
Note: In case of failure to grant the visa, the amount paid is not under any circumstances refundable.
