DOCUMENTARY REQUIREMENTS

Passport
Signed passport with at least 2 blank pages
Immigration Officers require it to have at least 6 months validitycompleted and signed
PSA Birth and Marriage Certificate (married applicants)
Make sure that the certificate is issued within 1 year
Completed travel itinerary with travel plan and accommodation details
If applicant will shoulder all/part of the travel expenses
Bank Certificate with 6 months average daily balance. If the bank certificate does not have an ADB, bank statements within the last 6 months must be submitted.
Income Tax Return. Signed BIR Form 2303 for employees. For business owners, actual payment of the tax must be submitted together with the ITR.
